Service Description:
|
Access to health care support at home, at school or in the community * care coordinators work with people in the community or leaving hospital to determine what help is needed and arrange for care * care is provided in community settings such as the home, school, residential facility or community clinic * provides information about other community services and refers when appropriate

- personal support
- medical supplies and equipment
- connecting to other community support services and agencies
Additional services - specialized services may include:- access to assisted living and supportive housing
- access to respite care and convalescent care (short-term stays and adult day programs)
- access to long-term care homes, rehabilitation and complex continuing care beds
- school health support services to school aged children
- community support for people living with cancer
- hospice palliative care

Health Care Connect
- Refers people who do not have a doctor to a family health care provider who may be accepting new patients * once registered, program participants are assigned to a care connector employed by the Home and Community Care * care connectors work with family doctors and nurse practitioners who are accepting new patients
