Service Description:	Provides permanent homes for children and youth who cannot remain with birth families assists birth mother in making informed choices and offers support before and after birth conducts home studies on prospective adoptive families offers support to individuals waiting to adopt a child and after the child joins the family assists in locating non-identifying information with the Adoption Disclosure Register assists with searches to contact a birth relative and offers support if a reunion is requested
