N'Mninoeyaa Aboriginal Health Access Centre - Mamaweswen, North Shore Tribal Council- Nurse case managers provide support to older adults and adults with disabilities who require long term care and personal support after regular work hours and on weekends * coordinates community care services to meet at-home health care needs upon discharge from hospital * includes personal support, homemaking, and respite services
