Service Description:	Offers diabetes education and self-management support in office Diabetes education nutrition diabetes management resource materials outreach activities foot assessments and high risk foot care by Chiropody answer questions Outpatient services diabetes management nutrition calculated diets there is no waitlist assists patients to access funding (forms) * may provide samples
