Coordinates and delivers activities that provide a contact to a client on a regular basis to check the health safety and social needs of the client * contact can be through a phone call or face-to-face visit

Members of one of the participating First Nations * client may be an isolated older adult, physically disabled adult, people with Alzheimer's disease or other dementias or other significant disease
