Service Description:

Community-based case management for people with serious mental illness and substance use issues who are being released from jail. The program includes coordination of required services, discharge planning and referral to community resources before their release.

Referrals to the program can be made by police services, correctional facilities, organizations involved in legal and justice planning, or self by completing a referral application available on the website.
