Service Description:	This program assists with assessing client needs, developing, implementing, and reviewing service plans, and working with other community resources in meeting/achieving the individual's service needs. Support offered: connect individuals to resources, and advocacy assess and address substance use concerns develop and monitor service plans working with community collaterals to service indivudual's needs assist individuals to achieve goals and objectives
