Service Description:

Individuals living with complex medical and mental health conditions who may also have an acquired brain injury, or addiction issues are supported in this program.
Individuals involved in this program may have frequent involvement in withdrawal management, police, or emergency services.
On-site services include:

personal support workers

Application:

Applicants must be residing in the District of Nipissing and receiving case management services. Individuals must be capable of making their own decisions, participate in service planning, and provide proof of income. Applications will be accepted from other professional support staff in the community.
