Provides safe, professional in home health care for anyone who can't easily access * works with other community agencies to find solutions to health care needs with the goal of in home independence * Services include:
wellness/safety checks
post hospital discharge follow up
point of care testing
wound treatment
long term care waitlist support
12 lead ECGs
disease education/information
medication check/review
Ontario Telemedicine Network (OTN) appointment support
remote medical monitoring
referrals for healthcare services
