Service Description: Help indigenous patients with cancer access health care support and services at home * co-ordinate traditional and non-traditional community resources and healing * counselling and support before, during and after clinical appointments * liaison/support between the care team, patients and families * advocacy on behalf of patients * assistance with care planning * advocacy for end of life care focused on the individual's goals and wishes
