focus of program/functional goal: stabilization and progression
average length of stay: 40 days
interprofessional team: Geriatrician, Physicians, Nurses, Occupational Therapists, Physiotherapists, Recreation Therapist, Dietitian, Social Worker, Chaplain, Rehabilitation Assistants, Recreation Therapy Assistant, Geriatric Psychiatrist (on consult), Speech Language Pathologist (on consult), Physiatrist (on consult)

Adults 18 years old and over long-term illnesses/disabilities, requiring ongoing medical/nursing support that cannot be met at home or in a long-term care setting * on admission, may have limited physical and/or cognitive capacity due to medical complexity but are believed to have restorative potential
