Outreach to local methadone clinic is available.
Discharge planning is provided, as is advocacy, monitoring, and support.

Self-referrals or community organization referrals are accepted.
Call or walk-in during operational hours to complete a referral.
Must be enrolled in an opiate substitution program locally.
