Service Description: Following a community-based model, the workers provide a range of services for individuals who are experiencing a serious mental illness, who are currently in custody, and who require assistance reintegrating into the community once released from custody. Supports are offered to enable clients to live their optimal quality of life in the community according to their own needs and choices.

Application: Self-referrals accepted however most referrals are typically submitted through the custodial facility, but can be submitted by other service providers, organizations, police, family, and community as well as individuals themselves. Call for information.
