Time-limited, coordinated interprofessional rehabilitation plan of care at a low intensity through a combined and coordinated use of medical, nursing and allied health professional skills

interprofessional team: Geriatrician, Physicians, Nurses, Occupational Therapists, Physiotherapists, Recreation Therapist, Dietitian, Social Worker, Chaplain, Rehabilitation Assistants, Recreation Therapy Assistant, Geriatric Psychiatrist (on consult), Speech Language Pathologist (on consult), Physiatrist (on consult)
