As Registered Nurse, Telehomecare (Status: Full-Time, Start date: April 6, 2026; Location: Any Ontario Health atHome office within the North East region) you provide remote patient monitoring, health coaching, and disease-specific education to patients with complex chronic diseases, including Chronic Obstructive Pulmonary Disease and Heart Failure. Working in collaboration with the patient’s circle of care, the Telehomecare - RN focuses on enabling the patient to better manage his/her chronic disease through the support of RN monitoring and coaching.

What will you do?

  • Develops and implements care plans with the patients;
  • Assists patients in establishing self-management goals;
  • Remotely monitors patients and responds to alerts using established clinical protocols and guidelines;
  • Delivers patient education and conducts regular health coaching sessions with patient;
  • Participates as a member of the Telehomecare Team to provide input into the CQI (continuous quality improvement) processes for patient enrolment, monitoring, education, coaching and discharge;
  • Coordinates care with the Care Coordinator, caregiver, hospital, interdisciplinary team, Diabetes Education Centre, Primary Care Provider and other providers when required;
  • Establishes and maintains effective relationships with external service providers, primary health care professionals, and appropriate agencies to ensure health care services and resources are available when needed.

 

What must you have?

  • Registered Nurse in good standing with the College of Nurses of Ontario;
  • Strong knowledge base and relevant experience working with patients with complex chronic diseases (e.g. CHF, COPD, Diabetes, and Mental Health Disorders);
  • Current competencies in chronic disease management and patient self-management;
  • Adult education, health coaching, patient self-management and case management skills;
  • Strong critical thinking and problem solving skills;
  • Effective communication and listening skills;
  • Demonstrated ability to use technology and best practices to remotely monitor patients;
  • Ability to multi-task and work in a fast paced environment;
  • In-depth knowledge of home health care and other community resources within Ontario Health atHome;
  • Strong collaboration skills to manage service delivery in conjunction with care coordinators, patients, their family members/caregivers/SDMs, and other service providers;
  • Ability to deal with demanding interpersonal situations and respond with good judgment and understanding;
  • Working knowledge of computer software and Microsoft Office applications;
  • Must have valid driver’s license and access to a vehicle;
  • Advanced oral and written proficiency in English is essential.
  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.

Salary:  Hourly wage of $41.29 to $51.13
