Provides services to individuals with an intellectual disability such as:
- develop and follow up on a person centered plan
- help individuals adapt to a new environment
- develop support teams and linkages with other community resources
- provide services for interim case management for external/internal referrals
- help individuals transition from a home to an independent environment
- help individuals manage their funds, accessing other relevant services, etc
