Service Description:

Community-based case management for individuals with serious mental illness and substance use issues who are in conflict with the law for minor offences.
Program includes:
ongoing assessments
treatment planning
coordination of required services
individual, group and family counselling
conflict resolution and crisis prevention
housing supports
discharge planning including referral to community resources
court diversion order coordination
